Total prostatectomy: When is it appropriate?

Total prostatectomy is considered in cases of:

  • Localized prostate cancer: If the cancer is confined to the prostate and has not yet spread to surrounding tissue, total prostatectomy is often suggested as a curative treatment.
  • Aggressive tumors: For fast-growing prostate cancers or those at high risk of spreading, surgery may be recommended.
  • Failure of other treatments: If other treatments, such as radiotherapy or hormone therapy, are ineffective or no longer appropriate, surgery may be considered.

When is a total prostatectomy not recommended?

If prostate cancer has spread to other organs (metastasized),total prostatectomy is generally not curative and other treatments are preferred.

For men with a very short life expectancy due to other serious illnesses, the risks of surgery may not be justified.

Finally, each situation is unique.  Some patients may prefer other treatments (radiation therapy, hormone therapy, cryosurgery) or active surveillance, depending on their preferences and health profile.

Types of radical prostatectomy

Surgical treatment of prostate cancer is primarily based on radical prostatectomy. This can be done using three main techniques:

  • Open prostatectomy: A traditional procedure in which an abdominal incision is made below the belly button to access the prostate. It offers great precision but is associated with a longer recovery time.
  • Laparoscopic prostatectomy: A minimally invasive technique that uses small incisions and specialized instruments. It reduces postoperative pain and speeds recovery.
  • Robotic prostatectomy: An evolution of laparoscopy that uses a robotic system for greater precision and visualization. It is especially indicated for complex tumors.

Preparation for radical prostatectomy in Turkey

Before your radical prostatectomy, several consultations will be arranged in Turkey to prepare you as well as possible.

Total prostatectomy: Step by step

Total prostatectomy is performed under general anesthesia. During the surgery, the urological surgeon completely removes the prostate and the seminal vesicles (small glands located nearby). The urethra is then reconnected to the bladder to ensure normal urinary continence.

In some cases, a lymph node biopsy may be performed: the urologist will also remove some lymph nodes to check for the presence of cancer cells.

The sample taken during surgery will be analyzed in the laboratory to determine the exact type and stage of prostate cancer. This information will help your healthcare team determine if further treatment is needed.

To best preserve your ability to have an erection, your surgeon will try to preserve the erectile nerves. However, their preservation will depend on the location of your tumor.

When you wake up and for the first few days

After the surgery, you will wake up in a monitoring room. It is normal to feel tired and sometimes a little nauseous due to the anesthesia. Do not hesitate to report any pain to the nursing staff.

A temporary urinary catheter will be placed to aid healing. Drains may also be placed to drain any excess fluid. These devices will be removed as soon as possible. To prevent blood clots from forming, you will be given anticoagulant medication and wear compression stockings.

Side effects specific to prostatectomy
